NutriZou Informed Consent
NutriZou is an evidence-based nutrition coaching service performed by University of Missouri students in the Coordinated Program in Dietetics and supervised under the NutriZou Registered Dietitian-Nutritionist.
NutriZou is designed to help Missouri community members meet their nutrition-related goals through nutrition education and coaching. All information and practices use general healthy eating nutrition guidelines per the 2020-2025 Dietary Guidelines for Americans for healthy adults. If you have a food-related medical condition, we may refer you to an appointment with a Registered Dietitian if we deem appropriate.
What to Expect
NutriZou nutrition coaching includes 3 individual sessions with your nutrition coach: 1 initial nutrition assessment of 60 minutes + 2 follow up sessions of 45 minutes each.
The first session includes a discussion of your individual nutritional goals, which includes some questions about your health. Then you and your nutrition coach will decide the best course of action to meet your goals.
During the first follow up session, you and your coach will review your goals, modify them as needed, and create new goals for the following week, building towards your ultimate nutritional goal.
Food records have been proven to be one of the most effective tools in changing one’s dietary practices. You and your coach will discuss the use of these logs and how they might support your nutritional goals. These logs are simply a way to collect data, and there is no judgement involved with how they are completed. If you prefer to not use food records, we will use alternative methods.
This Informed Consent (“Consent”) answers some important and frequently asked questions concerning the nutrition coaching services provided by NutriZou and sets forth your agreement to receive these services. If you have further questions after reading this Consent, or other concerns not covered here, feel free to discuss them with your nutrition coach. We will provide a copy of this Consent to you if requested. The original copy will be kept in your nutrition coaching record. It is very important that you read the entire Consent carefully before signing. By signing this Consent, you are agreeing to participate in nutrition coaching services with NutriZou and agree to our Policies.
NutriZou Policies and Procedures
Please note: with your permission, we may record sessions for training and supervision purposes. The recordings will not include any of your identifying information, and will be permanently deleted after training and supervision activities are completed. NutriZou nutrition coaches are not permitted to work with clients who come to scheduled appointments under the influence of drugs or alcohol. If you arrive at the NutriZou Clinic intoxicated or otherwise unable to participate in treatment, we reserve the right to cancel or reschedule your appointment. If this occurs chronically, we may terminate services to you.
Confidentiality
To the extent required by law, we will protect the confidentiality of the information you provide to your NutriZou coach and of your records maintained by NutriZou. Your information and records will only be disclosed as required or permitted by law unless you authorize their release. With your permission, we may provide your name and contact information to other providers to refer you for services as needed.
